    Breast Augmentation is a popular procedure in Atlanta and all around the world. Breast augmentation allows patients to achieve fullness and projection while offering patients a variety of options to help ensure their unique goals are reached. From implant type, to implant size, where the implant is placed and even where the incision is made, each breast augmentation is tailored perfectly to suit the patient.     The goal was simple:  Improve recovery from breast augmentation procedures.  General anesthesia using inhaled gas and a breathing tube (endotracheal tube or LMA) is an incredible tool that allows the plastic surgeon to perform invasive procedures without the patient feeling or being aware of pain.  I have performed, and continue to perform a number of breast augmentations under general anesthesia and I consider it extremely safe .  But there are two major drawbacks to general anesthesia: 1. Many patients are so scared of general anesthesia that they change their mind and decide not to have breast augmebabantation, and 2. General anesthesia can leave the patient listless, groggy, and often nauseated a few days after surgery. By performing breast augmentation under IV sedation we can alleviate many of the fears patients have regarding general anesthesia and improve their recovery from surgery.  The medications used during IV sedation breast augmentation are strong sedatives and pain relievers that carry the added benefit of being short-acting.  A submuscular or dual plane breast augmentation procedure takes between 30-45 minutes to complete, such that I can finish the procedure just as the meds are wearing off.  We limit the amount of IV narcotic sedatives used, such as fentanyl or morphine, and this significantly improves the risk of postoperative nausea.  During breast augmentation under general anesthesia or IV sedation I inject 200-400cc of tumesecent fluid per breast.  This tumescent fluid contains long- and short-acting agents (lidocaine) which numb the tissues and it contains epinephrine, a strong vasoconstrictor, which prevents bleeding during and after surgery.  Proper infiltration of tumescent fluid beneath the muscle and around the breast is the key to performing breast augmentation under IV sedation.

    The cosmetic results of breast augmentation under general versus IV sedation are identical.  The type of anesthetic (general versus sedation) does not limit or change the plastic surgeon’s ability to create a precise submuscular pocket and to release the inferior and medial borders of the pectoralis muscle to place the breast implant in the correct position.  We have seen less postoperative nausea and much quicker surgical recovery from IV sedation as compared to general anesthesia.  When asked, our patients resoundingly report that they would prefer surgery under IV sedation.   We enjoy offering patients the option of IV sedation for breast augmentation, especially for those who are concerned about general anesthesia and for those who desire a quicker recovery from surgery.

    This patient had lost volume in her breasts due to aging and pregnancy. Although this would usually be thought of as a case suited to a breast lift, due to there being a low amount of sag in this patients breast we were able to achieve the results she desired with a simple breast augmentation.     Breast augmentation has long been the most popular procedure in plastic surgery as well as being a hugely important step in many gender reassignment cases. In transgender breast augmentation cases the choices of implant types and sizes is exactly the same, with the same beautiful results expected, but there are some points to take in to consideration.

    Typically in male to female transgender cases the patient has broader chest and shoulders that will accommodate larger implants, smaller implants may look too narrow and appear unnatural. As well as a wider chest, nipples will typically be lower on  a male chest than a females so the natural breast crease will often have to be lowered so that the nipple is more centered on the implant.

    Another aspect that is important during consultation is whether the patient is currently using hormone therapy. Patients who have been using hormone therapy will tend to have developed breast tissue and this can be associated with ptosis (or drooping) that would have to be taken in to account when planning the procedure. Many transgender patients are good candidates for trans-axilalry augmentation where the incision is made in the armpit because of the lack of ptosis of the breast tissue.

    Finally it is important for the patient to pay close attention to instructions from their surgeon to allow the implants to ‘drop’ in to a natural position on the chest. Because the pec muscle is generally larger in transgender patients the muscle has a tendency to hold the implant higher on the chest wall, with massage and compression the implant will fall in to a lower and more natural place on the breast.

    Plastic surgeons and reality television shows popularized the term ‘mommy makeover’. It’s a common Google search term and there are over 3,000 Instagram posts with the hashtag #mommymakeover.

    So what is it?

    After breast augmentation surgery, like any surgery, it’s always important to take it easy and follow your surgeon’s recovery instructions. But, once you are fully recovered, healed, and ready to get back to your work out routine (check with your surgeon about just how long you should wait) what precautions should you take? After all, you could now be jogging with large breasts for the first time in your life!

    You’ve probably seen photos of bodybuilders who have undergone breast augmentation so you know they aren’t skipping their workouts! Of course, while you get used to your new assets things are going to feel strange. The extra weight on your chest and the movement of the implants, particularly during arm exercises. Low-impact cardio is a great way to ease back in to exercise while you get used to your new implants.It is important to wait before resuming any chest exercises though, particularly with implants placed sub muscularly the muscle is healing so it is important to treat it as such.

    silicone breast implantFrom Dr McCluskey:  Many breast augmentation patients are told by their doctors to avoid exercise during the first few weeks after surgery.  I actually encourage patients to resume low-impact exercise (walking on treadmill or elliptical) within a few days after surgery.   I find that the most active patients feel better, have less pain, recover faster, and resume normal activities faster.  That said, I prefer for breast augmentation patients to wait a few weeks before resuming upper body workouts, particularly if they use heavy weights.  The pectoral muscles are partially released during breast augmentation surgery and this area needs to heal before strenuous exercise is resumed.  So the take-home message is that I want my breast implant patients to get out and walk or do light exercise very soon after surgery, but hold off on upper body weight lifting or pushups for a few weeks.
