    Tag Archives: Breast Augmentation

The Mommy Makeover is a popular procedure that helps women return to their ‘pre-baby body’ by combing procedures that address each individuals concerns. Even with a healthy lifestyle including balanced diet and exercise there are some changes that childbearing causes that cannot be reversed without surgery. Pregnancy creates abdominal wall separation, sagging skin and stretch marks that can be reduced with a Tummy Tuck. Fat deposits and problem areas can be remedied using liposuction. Breastfeeding also causes reduced breast volume and sagging that can be addressed using either Breast Augmentation or a Breast Lift.

This patient received a beautiful transformation by combining a Tummy Tuck, Breast Augmentation and Liposuction with Dr McCluskey. As you can see below there is greater waist definition, fuller breast volume and a flat stomach that would have been impossible through diet and exercise alone.

    Breast Augmentation Plastic Surgery

breast augBreast augmentation or simply known as breast implants are used to increase the breast size in order to achieve post mastectomy reconstruction in case of breast cancer treatments or purely for cosmetic appeal. Breast implants are classified according to the type of implant, the treatment procedure or even the resulting complications.

    Types of breast implants

There are two major types of breast implants, they are saline and silicone implants. The former is made of silicone shells filled with sterile salt water (saline) and the latter is made of silicone shells filled with silicone (plastic gel). Both saline and silicone implants are available in different profiles to suit the projection goals of each patient. Silicone implants are available in the traditional round implant shape aswell as the shaped ‘gummy bear’ implants that more closely mimic the natural shape of a breast.

Most patients have accepted that silicone based implants feel more natural. Care must be taken to ensure that they are prevented from leaking as it can pose some serious risks. Silicone implants were banned in the year 1992 due to safety concerns but were re-introduced in 2006 after favorable scientific research. A rupture in a silicone implant doesn’t lead to deflation as the gel will remain trapped inside the capsule tissues. There, silicone rupture is also referred to as silent rupture.

Ruptured silicone implants are bound to cause deflation which results in irregular shapes and contours of the breast. The solution leakage will be absorbed naturally into the body but doesn’t lead to any health complications. The empty shell must be removed or replaced by a surgery.

There common risks posed by both type of implants such as breast pain, occasional uneasiness, rupture and capsular contracture.

    Implant safety

Both saline and silicone implants have been tested and certified to be safe, but they may not last long. Ruptures in saline implants can be spotted easily when changes in the breast shape are recurring. Whereas, in silicone implants ruptures are hard to spot due to the cohesive gel not separating. Even though they do not deflate as saline implants, it is suggested to surgically remove or replace ruptured silicone implant.

Silicone implants come pre-filled to a standard size before they are inserted. Saline implants are filled after they are inserted through a smaller incision. That also allows the saline implants to be slightly adjusted even after the surgery. In order to ensure maximum safety regarding the use of implants certain eligibilities have been imposed. In case of reconstruction, either type of implants is permissible for patients of any age. In augmentation, saline implants are permissible for those above 18 and silicone implants for those above 22.

    Cost

The cost of breast implant can vary according to the type of implant, the hospital or the doctor’s reputation. Typical range of breast augmentation cost at the Plastic Surgery Institute of Atlanta is $3000 to $6,000. As in case of any cosmetic surgery, breast augmentation isn’t covered by insurance.

The goal was simple:  Improve recovery from breast augmentation procedures.  General anesthesia using inhaled gas and a breathing tube (endotracheal tube or LMA) is an incredible tool that allows the plastic surgeon to perform invasive procedures without the patient feeling or being aware of pain.  I have performed, and continue to perform a number of breast augmentations under general anesthesia and I consider it extremely safe .  But there are two major drawbacks to general anesthesia: 1. Many patients are so scared of general anesthesia that they change their mind and decide not to have breast augmebabantation, and 2. General anesthesia can leave the patient listless, groggy, and often nauseated a few days after surgery. By performing breast augmentation under IV sedation we can alleviate many of the fears patients have regarding general anesthesia and improve their recovery from surgery.  The medications used during IV sedation breast augmentation are strong sedatives and pain relievers that carry the added benefit of being short-acting.  A submuscular or dual plane breast augmentation procedure takes between 30-45 minutes to complete, such that I can finish the procedure just as the meds are wearing off.  We limit the amount of IV narcotic sedatives used, such as fentanyl or morphine, and this significantly improves the risk of postoperative nausea.  During breast augmentation under general anesthesia or IV sedation I inject 200-400cc of tumesecent fluid per breast.  This tumescent fluid contains long- and short-acting agents (lidocaine) which numb the tissues and it contains epinephrine, a strong vasoconstrictor, which prevents bleeding during and after surgery.  Proper infiltration of tumescent fluid beneath the muscle and around the breast is the key to performing breast augmentation under IV sedation.

The cosmetic results of breast augmentation under general versus IV sedation are identical.  The type of anesthetic (general versus sedation) does not limit or change the plastic surgeon’s ability to create a precise submuscular pocket and to release the inferior and medial borders of the pectoralis muscle to place the breast implant in the correct position.  We have seen less postoperative nausea and much quicker surgical recovery from IV sedation as compared to general anesthesia.  When asked, our patients resoundingly report that they would prefer surgery under IV sedation.   We enjoy offering patients the option of IV sedation for breast augmentation, especially for those who are concerned about general anesthesia and for those who desire a quicker recovery from surgery.

Following weight loss, breast feeding, and simply with age, breasts lose volume and begin to sag. Losing that youthful full bust is not uncommon, but is a factor in reduced confidence that we see in many of our patients. Breast implants are a great solution to this loss of volume and returning the breasts to a more rounded, youthful shape.

As can be seen below, in this patients case breast implants can be used to create a very natural looking, full chest while in many cases increasing self-esteem and confidence. There are so many options available for breast implants, from material, to size, to placement each patient is able to have their specific and unique needs met. Dr McCluskey discusses all options in detail with each patient so that the procedure is tailored to meet expectations as well as possible.breastaugba
