    Reaction To The Bruce Jenner Interview

    I never thought I would hear Bruce Jenner admit to the world that he’s always identified as a woman.  I still remember the Bruce Jenner of the late ’70’s and ’80‘s, the personification of masculinity.  He was the all-American golden boy and although I was very young at the time, I looked to him as a hero.  tumblr_nnck7yBU5d1sy2327o1_500

    There are those who have said that his sudden interest in being female is just another tacky publicity stunt by the Kardashian media empire. But nothing about his interview with Diane Sawyer this week appeared gimmicky or staged.  Mr Jenner was honest and clearly he has been struggling with this issue for many, many years.  In his interview he discussed cosmetic surgery and going forward with physical changes to achieve a more feminine look.  While I was once among those who thought that his facelift, eyelid lift, and rhinoplasty were all somewhat drastic and overdone, it is now clear that he was seeking a more feminine appearance.

    I’ve been treating transgender patients for many years now.  We welcome them into the practice and we offer many feminizing procedures for the face and body.  I remember my first consultation with a transgender patient, and I’ll freely admit that it was unfamiliar territory.  The patient could tell that I was not accustomed to discussing breast augmentation and facial feminization procedures with transgender patients.  The first hurdle for me was accepting that this was indeed a female, “she”, and the second was moving past her obvious male physical characteristics and talking to her in the same way I would an anatomically female patient.  But this patient understood and she was open and honest with me. She had been on feminizing hormones since the age of 16 and she had “B” cup breasts and soft features.  Ultimately I performed breast augmentation on that patient and she has continued to refer her friends and family to the practice for years.

    From those first few patients I’ve established a great following in the transgender community and the patients literally travel from all over the world to our office in Atlanta.  Through the transgender community I’ve gained invaluable experience with facial feminization procedures such as facelift, eyelid lift (blepharoplasty), necklift, browlift, and rhinoplasty.  But facial feminization also involves softening the face with procedures such as cheek augmentation, chin and jaw reduction, glabellar (brow) reduction, lip augmentation, and buccal fat removal.  In recent years we have improved techniques for injecting autologous fat and fillers such as Restylane, Radiesse, and Juvederm to aid with feminization.

    Unfortunately many transgender patients are turned away by plastic surgeons and cosmetic surgeons.  Many doctors are under the impression that these patients have unrealistic expectations or that they want to transform their male appearance into that of a Barbie doll.  I don’t find that to be the case at all.  I find that most transgender patients have very reasonable expectations for surgical results.  They want what every cosmetic surgery patient wants, to look like a better version of themselves.  When transgender patients are rejected by medical practices they often turn to dangerous procedures such as black market injection of industrial grade (non-medical) silicone, PMMA, and other substances. These injections are frequently performed in hotel rooms or in people’s houses or apartments and there is no guarantee as to what type of substance is being injected.  The clear liquid in those syringes could be industrial grade silicone or it could be tile caulk or even tire filler.  Many of my transgender patients have had these substances illegally injected into their faces, cheeks, lips, breasts, and buttocks with terribly deforming results.  I’m often asked to remove these substances or to improve the appearance of the affected areas but unfortunately the treatment options are very limited.

    I hope Bruce Jenner’s revelation has a strongly positive effect on the transgender community.  I hope that it improves how transgender and transsexual patients are viewed as both people and as patients in medical practices.  We cannot prevent people from turning to black market treatments such as silicone injection, but if these patients felt more comfortable walking into a physician’s office and discussing their concerns then they would be much less likely to seek illegal body and facial modifications.

    Rhinoplasty (Nose Job) Surgery Atlanta | Plastic Surgeon | SurgeryFor people with darker skin pigments keloid scarring can be an ugly fact of life.  A keloid is an overgrowth of scar tissue at a site of injury.  Keloids can occur from trauma, burns, surgical incisions, lacerations, piercings, and even tattoos.  In extreme cases keloids can even form after a simple abrasion to the skin.  Imagine scraping your elbow or knee and forming a a large painful keloid scar afterward!  Again, this is an extreme example but this can happen.  The good news is that keloids are treatable.  The bad news is that it’s not always an easy fix and in many cases it requires multiple treatments and/or procedures.

    Not all unsightly or dark scars are keloids.  A widened scar is simply that, a scar which has stretched or expanded because it is over a moving joint (such as the knee or elbow) or because it’s in an area of tension.  Most widened scars are not keloids therefore they won’t be treated the same way keloids are treated.

    In a keloid scar the collagen which helps repair the cut skin edges grows too much and the scar will appear swollen, hard, and sometimes painful.  Because a keloid is first and foremost an inflammatory reaction one of the first-line treatments is a steroid injection.  Kenalog steroid is injected directly into the keloid scar and after a few weeks the scar will appear flatter and lighter in color.  If this injection is repeated every month then in most cases the patient will see a significant improvement in the keloid scar.

    In an ideal situation the keloid shrinks and softens after kenalog injections.  Often as it shrinks I am able to excise the keloid and re-close the incision.  Patients who have very bad or large keloids sometimes require a single dose of radiation therapy after excision to prevent the keloid from recurring.  This single dose of radiation has been very effective in preventing or slowing recurrence but it is only necessary for large or refractory keloid scars.

    Sedation Breast Augmentation

    The goal was simple:  Improve recovery from breast augmentation procedures.  General anesthesia using inhaled gas and a breathing tube (endotracheal tube or LMA) is an incredible tool that allows the plastic surgeon to perform invasive procedures without the patient feeling or being aware of pain.  I have performed, and continue to perform a number of breast augmentations under general anesthesia and I consider it extremely safe .  But there are two major drawbacks to general anesthesia: 1. Many patients are so scared of general anesthesia that they change their mind and decide not to have breast augmebabantation, and 2. General anesthesia can leave the patient listless, groggy, and often nauseated a few days after surgery. By performing breast augmentation under IV sedation we can alleviate many of the fears patients have regarding general anesthesia and improve their recovery from surgery.  The medications used during IV sedation breast augmentation are strong sedatives and pain relievers that carry the added benefit of being short-acting.  A submuscular or dual plane breast augmentation procedure takes between 30-45 minutes to complete, such that I can finish the procedure just as the meds are wearing off.  We limit the amount of IV narcotic sedatives used, such as fentanyl or morphine, and this significantly improves the risk of postoperative nausea.  During breast augmentation under general anesthesia or IV sedation I inject 200-400cc of tumesecent fluid per breast.  This tumescent fluid contains long- and short-acting agents (lidocaine) which numb the tissues and it contains epinephrine, a strong vasoconstrictor, which prevents bleeding during and after surgery.  Proper infiltration of tumescent fluid beneath the muscle and around the breast is the key to performing breast augmentation under IV sedation.

    The cosmetic results of breast augmentation under general versus IV sedation are identical.  The type of anesthetic (general versus sedation) does not limit or change the plastic surgeon’s ability to create a precise submuscular pocket and to release the inferior and medial borders of the pectoralis muscle to place the breast implant in the correct position.  We have seen less postoperative nausea and much quicker surgical recovery from IV sedation as compared to general anesthesia.  When asked, our patients resoundingly report that they would prefer surgery under IV sedation.   We enjoy offering patients the option of IV sedation for breast augmentation, especially for those who are concerned about general anesthesia and for those who desire a quicker recovery from surgery.

    The G-Shot at PSIAtlanta

    What is the G Shot?

    First, the G Shot is a simple, pain-free procedure which is performed in the office. Second, the G Shot provides a nice enhancement of the most sensitive spot on the vaginal wall. There are many theories about the location of the “G spot” but experts agree that the G spot is a patch of tissue on the anterior (upper) vaginal wall which is often more prominent than the surrounding tissue. Many of the nerve endings associated with sexual pleasure are concentrated in the clitoris and the G spot. Some women don’t experience much sensation in the G spot while others find it to be a very sensitive area. The G Shot works best in women who already experience heightened sensation in the G spot area. Enhancing the volume and prominence of this area on the vaginal wall improves sensation for both the woman and her partner.

    How does the G Shot work?

    I completely understand that it’s a little intimidating and embarrassing for a patient to talk about these procedures during a consult. Patients have a lot of questions about procedures like the G Shot, or for that matter, anything to do with sexual aesthetics & vaginal rejuvenation, but they’re often afraid to ask. We’ve worked very hard to make this experience as comfortable and un-intimidating as possible. Often for procedures like the G Shot I will schedule the consult and the procedure on the same day. This means just one trip to the doctor! This means just one quick discussion about the procedure and what results the patient is trying to achieve, then a little numbing medicine, a little G Shot, and she’s out the door within the hour! In short, the G Shot is an injection of filler into the G spot.

    Use of Fillers For The G-Shot

    I typically use hyaluronic acid fillers such as Juvederm but I’ve performed a number of G Shots by transferring the patient’s own fat into the G spot area. Both of these fillers (fat and hyaluronic acid, or HA) have their advantages and disadvantages. The hyaluronic acid G Shot is the simplest of in-office enhancements and only takes a few minutes. The patient can easily be in and out of the office in 20-30 minutes and the HA enhancement can last 9 months or longer. A G Shot with HA can also be repeated months later for further sexual enhancement without any risk of scar tissue formation or other side effects. Hyaluronic acid is a naturally occurring component of the human body and the results of an HA G Shot actually improve with time because HA absorbs water causing additional prominence of the G spot.

    Using The Patient’s Own Purified Fat For A G-Shot

    Performing a G Shot with the patient’s fat takes a few extra minutes as compared to HA but the results are excellent. G spot enhancement using the patient’s own fat will also last 9 months or longer and it can be repeated to improve the desired effect. Another advantage of using fat is that it can easily be combined with other procedures. If we are performing liposuction of other areas why not transfer some of the fat to the G spot to improve sexual satisfaction? The G shot only requires a few cc’s of fat (1-5cc) but during a typical fat-harvesting procedure I’ll remove 20-30cc of fat. We call this fat “liquid gold” because it is the patient’s own fat, dense in tissue factors and stem cells, and it can be used to enhance multiple areas of the body including the G spot, the labia majora, and labia minora, facial lines & lips, and it can be used for rejuvenation of the hands.

    To reiterate, the G shot is a simple, short, pain-free, in-office procedure which can be performed on the same day as consultation. Patients will often experience heightened sensation afterward with very few, if any, risks or complications.
