    Working Out After Rhinoplasty

    When you are recovering from rhinoplasty and busy admiring the beautiful results, you’ll probably feel inspired to feel even better about yourself by working out and eating right! Besides wanting to protect your brand new nose, when you’ve just had a rhinoplasty the idea of cardio and heavy breathing sounds like a nightmare! So where do you begin?

    During the first 2 weeks after rhinoplasty patients should avoid strenuous activity and sleep in a slightly inclined position.  This period of recovery is important because the nose has to heal and you want to minimize swelling.  But active patients won’t tolerate too much downtime.  Assuming the nose is healing nicely, rhinoplasty patients may begin brisk walking & light exercise two weeks after surgery.  One month after surgery patients may resume vigorous exercise and heavy lifting.

    Fame and the Recognition of Doctors

    I loved reading the article “A Pioneering Force” in this month’s Town & Country (October, 2013) magazine.  The article highlights the career and accomplishments of Dr Kristi Funk of Beverly Hills.  Dr Funk is a breast surgeon who performed a double mastectomy on Angelina Jolie.  Like many doctors who treat celebrities, Dr Funk is bound by patient confidentiality laws and would have never revealed the identity of her superstar patient.  But as the article reveals, Angelina Jolie described Dr. Funk’s Beverly Hills clinic to the New York Times and effectively changed the course of the breast surgeon’s career.  This is the kind of press that surgeons such as Dr Funk deserve.  As a film actress, fame is largely an indicator of Angelina Jolie’s success.  But in medicine, accomplishment and fame do not go hand in hand.  The most famous medical doctors are those who find their way onto television, whether as talk show hosts or reality show spoofs.  The most accomplished doctors who do ground breaking work day after day will never gain attention outside of their own professional societies.  It’s highly unlikely that the average patient has any idea who performed the first heart transplant, or from a plastic surgery perspective, the first face or hand transplant.  But ask the average patient about famous plastic surgeons and they’ll likely name the entire cast of “Dr 90210” or a plastic surgeon’s wife from one of the infamous “Housewives of…” shows.   In the field of plastic surgery the doctors on the E! television hit “Dr 90210” became household names.  As I was finishing my training at UT Southwestern in Dallas one of our rhinoplasty patients told me she had only come to see my boss, Dr Rod Rohrich (Chairman in Plastic Surgery at UT Southwestern, editor-in-chief of the Journal of Plastic & Reconstructive Surgery, past President of the American Society of Plastic Surgery, and editor/author of the Dallas Rhinoplasty text) because the doctors on “Dr 90210” did not have any available appointments.  I explained to her that Dr Rohrich was one of the top rhinoplasty surgeons in the world, one of a handful of experts whose opinion and surgical techniques are valued and studied by most, if not all surgeons who learn rhinoplasty.  She responded that she had indeed heard great things about Dr Rohrich but had she really wanted to go to a famous plastic surgeon such as those on “Dr 90210.”  Just to be clear, I have nothing against those doctors who participate in reality television or talk shows.  In fact, for better or worse, they promote the field of plastic & cosmetic surgery  and help people feel more comfortable about discussing topics such as liposuction or breast augmentation.

    With regard to Dr Funk and Angelina Jolie, prophylactic, or preventative mastectomy has been performed for many years on patients at high risk of developing breast cancer.  The numbers of patients undergoing these procedures have dramatically increased with improvements in breast reconstruction.  The Town & Country article states that Dr Funk used a nipple sparing mastectomy technique on Angelina Jolie.  It wasn’t that long ago that undergoing a mastectomy meant that a woman would forever have a long scar across one side of her chest and she would have to wear a prosthesis to look normal in clothes.  But through experimentation and rapid advancements the breast tissue can now be removed through a small incision that leaves the nipple intact.  The plastic surgeon then reconstructs the breast during the same procedure and the patient awakens from either a single or double mastectomy surgery with normal appearing breasts.  Imagine the psychological boost this must provide, especially if the patient has been diagnosed with breast cancer.  Now she can face her diagnosis without losing some aspect of her identity or her femininity.  These are the surgical accomplishments that should gain national attention.  So I applaud Dr Funk and the many doctors and surgeons out on the front lines of healthcare who worry more about their patients than their Google ranking or the number of social media followers they have.

    Profile Surgery

    Noses can be big and noses can be small.  Big noses can be tapered and refined through nose surgery, or rhinoplasty.  Likewise small noses can be broadened or lengthened.  But the nose is just one part of a person’s profile.  While the entire face contributes to the profile, the chin can determine the attractiveness of the profile.  Like noses, chins come in all shapes and sizes.  Chins can be broad and overly projected, or narrow and underprojected.  The good news is that these issues can be fixed!  Chin augmentation, which typically involves placement of a chin implant, can provide more width and projection to a slight chin.  Chin augmentation is one of the most common facial procedures and it can be safely performed under local anesthesia.  Like breast implants, chin implants come in a variety of sizes and widths.  To choose the correct size I take a number of measurements of the patient’s chin and jaw and then consider how much additional projection the patient needs or desires.  Often a chin implant is performed at the same time as rhinoplasty to treat the entire profile and restore harmony and symmetry to the face in a single procedure.

    Chins can also be reduced and or narrowed to improve the attractiveness of the profile.  Chin narrowing or chin reduction are feminizing procedures and are often performed in women who have broad, overly projected, or masculine appearing chins.  Chin narrowing is also performed as part of FFS (facial feminization surgery) along with rhinoplasty, cheek augmentation, glabellar and brow reduction, and tracheal shave procedures.

    I find that for patients with a slightly retruded chin, the placement of a chin implant under local anesthesia dramatically improves the overall attractiveness and symmetry of the face.  It’s an inexpensive procedure with minimal downtime & easy recovery that yields beautiful results.

    What Blepharoplasty Can Do?

    The patient should have clear goals, positive outlook and realistic expectations in order to achieve satisfactory results. The patient should be a non-smoker or have the commitment to quit smoking several weeks prior to the surgery. People with serious eye ailments may not make ideal or outstanding candidates for this procedure.

    Eyelid Surgery Procedure

    The cosmetic surgeon may decide to perform eyelid surgery under general anesthesia or IV sedation, depending on the circumstances of the case. Dr. McCluskey ensures that his patients in Atlanta, GA and nearby areas receive effective treatment in the safest possible manner.

    He will try to conceal the incisions within the natural structures of the eyelid region so that scars do not become obvious later on.     Lower eyelid anomalies may be corrected with an incision that is made just beneath the lower lash line. Excess skin in the lower eyelids can be removed through this incision. Redistribution or removal of excess fat in the lower eyelid can also be done through a transconjuctival incision. The incisions are finally closed with absorbable sutures, tape, or skin adhesive.

    Results

    Eyelid surgery results will appear gradually as the bruising and swelling resolves to show up a well-defined eyelid and surrounding area. Full effects can be seen after a few weeks, and the final results are fairly long lasting. The plastic surgeon will usually recommend lifelong sun protection to retain results for a longer time.

    Introducing Liberator Products for Postoperative Recovery

    1902939_552417368188424_585759038_nI’m very excited to announce our partnership with Liberator, inc.  Liberator is an Atlanta-based manufacturer of a line of extremely popular foam wedges and furniture.  More specifically, Liberator makes “sex furniture.”  While evaluating the Liberator products I saw incredible potential for their use during surgical recovery.  So many of the procedures I do require special positioning after surgery and these positions are difficult to comfortably achieve on a bed or sofa at home.  Liberator’s products are made of sturdy, comfortable but firm foam with washable, water-resistant covers.  This makes these products ideal for postoperative use.  For example, the Liberator Ramp, or Ramp/Wedge combo put the patient in the ideal flexed position after a tummy tuck.  The patient can comfortably rest with the head & torso elevated at 30-60 degrees and the addition of the smaller wedge beneath the knees supports the legs.  When I maneuver the head & legs of a hospital bed for a tummy tuck patient after surgery, this is the precise position I’m trying to achieve.

    The Liberator Hipster solves many of our post Brazilian Butt Lift concerns.  The Hipster has a unique shape that allows patients to lie prone comfortably.  The absolute most important thing patients must do after Brazilian Butt Lift is stay off their backside.  As much as possible, they must limit sitting and lying in a supine position.  The Hipster has gentle curves that allow patients to assume a number of positions without shifting their weight onto their buttocks.
