It has about 2,600 plastic surgeons as certified active members. All of them have been extensively trained in all areas of plastic surgery.

    The 18th annual multi-specialty statistical data was released recently by American Society for Aesthetic Plastic Surgery (ASAPS). It shows that the American are spending more than 12 billion dollars altogether on surgical and non-surgical procedures while there was about 10 million cosmetic surgery procedures performed by cosmetic surgeons in the year 2014. Most specifically the buttock augmentation went up by 86%, labiaplasty by 49% and non-surgical fat reduction by 42%. Trends indicate the willingness of men to undergo aesthetic cosmetic procedures.

    Top five surgical and non-surgical procedures

    Both men and women have undergone cosmetic surgery procedures in the year 2014. Specifically, Liposuction had about 342,494 procedures, Breast Augmentation about 286,694, Eyelid Surgery about 165,714, Tummy Tuck about 164,021and Nose Surgery about 145,909.
    Non-surgical procedures record more number of patients than surgical procedures. The top five non-surgical procedures are Botulinum Toxin with 3,588,218 procedures, Hyaluronic Acid with 1,696,621 procedures, Hair Removal about 828,480 procedures, Chemical Peel about 484,053 procedures, and Microdermabrasion about 417,034 procedures.

    There are some procedures, which also includes the ones in the top five, which underwent a decline in popularity. Breast augementation was down by 8.5%, ear surgery by 12.3%, injectable by 6.5% and lastly Photorejuvenation underwent a record low of 18.9%.

    Increase in number of male patients

    Yet, there has been a clear increase in the number of male patients for both surgical and non-surgical procedures. The total number of men undergoing cosmetic procedures has increased by a whopping 43% this year.

    The figures recorded from the past five years indicate the growing popularity of cosmetic procedures among males. Blepharoplasty had about 34% increase in procedures, Male Breast Reduction had about 33% increase, and Facelift became a popular procedure with 44% increase.
    Nonsurgical procedures have been more popular as well. Procedures like Botulinum Toxin had about 84% increase in the number of male patients, Hyaluronic Acid had about 94%, Nonsurgical Skin Tightening had about 37% increase and the Intense Pulsed Light had about 44% increase.

    Ever since the statistical data collection originated in 1997 for aesthetic surgeries, there has been a 273% increase in the number of males undergoing cosmetic procedures. The trend is expected to grow further in the coming years. It revolves around the fact that men, like most women, are also becoming conscious of their physical appearance. It is especially true of men in their middle ages. Various men who are celebrities also have undergone cosmetic surgical procedures to heighten their profile and set an example to the remained of men population who follow them.     Sedation for Vaginal Rejuvenation

    summary-of-the-american-society-for-aesthetic-plastic-surgery-reportI am always trying to improve our patients’ experience before, during, and after surgery.  As I’ve discussed with regard to breast augmentation many patients have a huge fear of general anesthesia. Because vaginal rejuvenation focuses on a very limited region of the body, the labia and vaginal tissues can easily be “numbed” or anesthetized locally while the patient is in a sedated state. The process is simple. Our anesthesia team will start an IV before surgery and administer sedation medications.  Patients enter what we call a “dissociative state” whereby they are still breathing on their own (without breathing tubes or the assistance of a ventilator) but they do not feel pain. These sedative medications are short acting and the anesthesia provider can quickly awaken the patient when the procedure is complete. Once the sedative medications are on board I inject Xylocaine (lidocaine with epinephrine) into the tissues. With the tissue adequately anesthetized with lidocaine I can safely peform vaginal rejuvenation, labiaplasty, G-spot augmentation, clitoral hood reduction, episiotomy (perineal body) repair, and autologous fat grafting while the patient rests comfortably. If a patient specifically requests general anesthesia, or if vaginal rejuvenation is being combined with a larger procedure such as a tummy tuck or breast lift, then that can easily be arranged. Recovery after sedation/local procedures is quicker and easier; the patients return to normal activities more rapidly and they aren’t burdened with the “fog” of general anesthesia for 3-4 days.

    Why Choose A Plastic Surgeon for Vaginal Rejuvenation?

    One of my colleagues recently showed me an excerpt from another doctor’s website.  This doctor is an OBGYN who specializes in vaginal rejuvenation and urinary incontinence (a urogynecologist).  This doctor’s website has an entire page explaining how OBGYN’s who did a fellowship in urinary incontinence have superior training to plastic surgeons as it relates to cosmetic vaginal rejuvenation and labiaplasty.  I have worked with a number of urogynecologists over the years and I’ve trained along urogynecology fellows and assisted them on complicated surgical cases.  In 20 years I’ve never heard a urogynecologist discuss cosmetic surgery such as vaginal rejuvenation or labiaplasty.  But I’m not very territorial.  I feel that an OBGYN has just as much right to perform these procedures as a plastic surgeon.  I do take issue when those same doctors disparage our plastic surgery training, and they do so with blatant ignorance.  The page from this OBGYN website contains a lot of false information about plastic surgery training and I’d like to clarify a few points.

    I have great respect for OBGYN’s and it is a shame that decreasing insurance reimbursement and worsening liability has pushed many OBGYNs away from delivering babies.  I understand the financial pressures of running a medical practice but I also firmly believe that doctors should stick to their training.  I cannot comment on the specifics of OBGYN training but from what I recall from medical school and residency 99% of their training involves obstetrics, reproductive medicine, gynecologic cancers, and other gynecologic disorders such as endometriosis, uterine fibroids, and hormone imbalances.  I’ve never associated OBGYN’s or urinary incontinence specialists (Urogynecologists) with cosmetic surgery or cosmetic procedures.  In Atlanta I’ve encountered numerous OBGYN’s who perform tummy tucks, breast lifts, and even breast augmentation and facial procedures including Botox, injectable fillers, and laser skin rejuvenation.  I’ve also repaired a number of botched cosmetic procedures by these OBGYN’s, including tummy tucks with the incision placed at or above the belly button, and liposuction with terrible contour deformities.  It is unfortunate that they feel the need to step so far out of their “realm of practice” (the specialty in which they trained) in order to make money.

    The OBGYN website to which I’m referring states:  “Plastic Surgeons – usually spend 3 years in general surgical training learning various surgeries including bowel surgery, chest surgery, appendix removal, gallbladder removal and then another 2-3 years in plastic surgical training where they spend most of their time doing wound treatment for ulcers and burns…”  This statement is 100% false.  I don’t know the source of their information (no sources are cited) but my training was typical of many plastic surgeons so allow me to take a walk down memory lane.  I completed 8 years of residency in surgery and plastic surgery.  The entirety of my training from the moment I graduated from medical school was devoted to surgery and the careful handling of tissue.  As a result of completing a full general surgery residency, like many of my peers, I have operated on every structure and tissue type in the body from head to toe.  I’ve performed these operations in both elective and trauma settings and I’ve literally participated in lifesaving surgical procedures, whether it be a stab wound to the heart, a ruptured aorta, a severed limb, or a log through the neck (yes, a 6-inch log through the neck, in a patient who arrived in the ER at 2am one night, and after a 4 intense hours of surgery and a few days in the hospital that man went home in good condition).  For five years, day after day, night after night, my colleagues and I learned proper tissue handling, wound healing principles, and surgical skills by operating constantly.  I’m from what you call the “old school” in that I trained before the limited 80-hour workweek.  Most of my weeks in surgical training extended 7 days and well beyond 100 hours and I never complained because this training prepared me for every possible surgical situation.

    All of that training landed me in a world-class plastic surgery residency program at UT Southwestern Medical Center in Dallas, Texas.  The focus of plastic surgery training is again on handling delicate tissues, wound healing, and refined surgical skill.  Without getting into the gory details, the field of plastic surgery encompasses cosmetic and reconstructive surgery of all parts of the body.  If a physician wants to become an expert in cosmetic surgery, whether that is breast augmentation, eyelid lifts, labiaplasty, or vaginal rejuvenation, then that physician will receive superior training as a plastic surgeon.  Likewise, if a physician wants to do Botox and injectable fillers should that physician become an OBGYN, or would she be more suited as a plastic surgeon or dermatologist?

    With regard to reconstructive surgery, an important aspect of plastic surgery training involves reconstruction of the vagina and vulva after removal of cancer.  Malignancy can often leave large defects and terrible deformities in these areas.  Using combinations of muscle flaps, skin and tissue grafts, and autologous fat transfer techniques, plastic surgeons can completely reconstruct these defects and deformities so that the patients can enjoy normal cosmetic vaginal appearance and normal sexual function.  Many of the cosmetic and functional techniques of labiaplasty and vaginal rejuvenation were in fact designed by plastic surgeons and these techniques were derived from the larger, more complicated reconstructive procedures.

    To sum it up, I do not perform urinary incontinence surgery because I am not a Urogynecologist.  I am not an OBGYN so I do not deliver babies.  If a doctor wants to perform cosmetic surgery then the proper route for training is plastic surgery.  If a doctor wants to fix urinary incontinence then the proper training route is OBGYN and Urogynecology.    That is not to discredit the training of a urogynecologist – I have witnessed these surgeons perform very elegant procedures for very complex problems.  Again, I’ve worked with OBGYN and Urogynecology department chairmen and experts in the field and I’ve never heard them mention cosmetic surgery as part of their training.  I’m sure that their skills can allow them to become adept at cosmetic procedures but it’s a pretty big leap to directly say on a website that plastic surgery training is somehow inadequate.

    Why do I perform labiaplasty and vaginal rejuvenation surgery?  These are cosmetic procedures for which I was specifically trained as a plastic surgeon.  My extensive background in general surgery and plastic & reconstructive surgery have more than amply prepared me to perform these procedures.  A major focus of my practice is also correcting “botched” procedures performed by other doctors or in other countries. Corrective or revision surgery of the labia and vaginal canal requires expertise in reconstruction as I discussed earlier.  I’ve refined a number of labiaplasty and vaginal rejuvenation techniques and I rely on my plastic surgery training and my surgical experience to consistently achieve excellent cosmetic and functional results.

Its popularity only continues to grow as more and more people are becoming aware of the benefits of quality plastic surgery by the hands of competent plastic surgeons .

    In 2013, the demand for some plastic surgery procedures grew while the demand for some other surgery procedures fell. The same trend continued into the year 2014, while some non-surgical cosmetic procedures also saw surge in popularity. The increasing clamor for ‘look good to feel good’ has driven thousands of Americans to undergo successful cosmetic procedures.

    Highest Charted Procedures

    Breast augmentation (Augmentation mammoplasty) had the highest number of patients. About 286,254 patients preferred breast augmentation. It also had the highest number of patients back in 2013 with 290,224 though the number has decreased by 1%. It was only the third highest cosmetic surgery procedure in 2000. The same trend is expected to continue in the year 2015. The increased quality of surgical and treatment procedures are attributed to the popularity of breast augmentation.

    Cosmetic surgeons found that nose reshaping (Rhinoplasty) became the second most popular cosmetic surgery procedure with a total of 217,124 patients. It had about 221,053 patients in the year 2013. So there is about 2% decrease in the patients for Rhinoplasty as compared to 2013. The same surgical procedure had about 389,155 patients in 2000, which means that the numbers decreased by 44% in the year 2014.

    There has been an increase in the number of liposuction patients for the year 2014. About 210,552 people underwent liposuction treatment. The number has drastically increased in comparison to previous year, where only 199,817 patients underwent liposuction. The number has increased by 5% this year. However, the numbers have dropped by 41% as compared to year 2000.

    Eyelid surgery (Blepharoplasty) had about 206,509 patients during 2014. The number has been decreased in comparison to the previous year when about 215,641 patients underwent the procedure. The number of patients has decreased by 4% in comparison to last year and by 37% in comparison to the year 2000.

    In 2014, face lift surgery (Rhytidectomy) had about 128,266 patients and the numbers appear to be slightly decreased from the previous year when there were 133,320 patients. It comes closer to the number of patients in 2000 during which about 133,856 people were treated.

    Totally, the year 2014 had about 1,677,478 patients undergoing various cosmetic surgery procedures. The number stays close to the number of patients from previous year, when there were about 1,668,420 patients who underwent cosmetic surgery. The popularity of cosmetic surgery seems to be declined as compared to the year 2000 when there were close to 2 million patients. So, the number of patients undergoing cosmetic surgery has decreased by 12%.

    Minimally invasive surgical procedures have been growing in popularity. There were about 13,945,388 people who underwent minimally invasive surgical procedure and it was a growth by 4% from last year and about 154% from the year 2000.

    Reaction To The Bruce Jenner Interview

    I never thought I would hear Bruce Jenner admit to the world that he’s always identified as a woman.  I still remember the Bruce Jenner of the late ’70’s and ’80‘s, the personification of masculinity.  He was the all-American golden boy and although I was very young at the time, I looked to him as a hero.  tumblr_nnck7yBU5d1sy2327o1_500

    There are those who have said that his sudden interest in being female is just another tacky publicity stunt by the Kardashian media empire. But nothing about his interview with Diane Sawyer this week appeared gimmicky or staged.  Mr Jenner was honest and clearly he has been struggling with this issue for many, many years.  In his interview he discussed cosmetic surgery and going forward with physical changes to achieve a more feminine look.  While I was once among those who thought that his facelift, eyelid lift, and rhinoplasty were all somewhat drastic and overdone, it is now clear that he was seeking a more feminine appearance.

    I’ve been treating transgender patients for many years now.  We welcome them into the practice and we offer many feminizing procedures for the face and body.  I remember my first consultation with a transgender patient, and I’ll freely admit that it was unfamiliar territory.  The patient could tell that I was not accustomed to discussing breast augmentation and facial feminization procedures with transgender patients.  The first hurdle for me was accepting that this was indeed a female, “she”, and the second was moving past her obvious male physical characteristics and talking to her in the same way I would an anatomically female patient.  But this patient understood and she was open and honest with me. She had been on feminizing hormones since the age of 16 and she had “B” cup breasts and soft features.  Ultimately I performed breast augmentation on that patient and she has continued to refer her friends and family to the practice for years.

    From those first few patients I’ve established a great following in the transgender community and the patients literally travel from all over the world to our office in Atlanta.  Through the transgender community I’ve gained invaluable experience with facial feminization procedures such as facelift, eyelid lift (blepharoplasty), necklift, browlift, and rhinoplasty.  But facial feminization also involves softening the face with procedures such as cheek augmentation, chin and jaw reduction, glabellar (brow) reduction, lip augmentation, and buccal fat removal.  In recent years we have improved techniques for injecting autologous fat and fillers such as Restylane, Radiesse, and Juvederm to aid with feminization.

    Unfortunately many transgender patients are turned away by plastic surgeons and cosmetic surgeons.  Many doctors are under the impression that these patients have unrealistic expectations or that they want to transform their male appearance into that of a Barbie doll.  I don’t find that to be the case at all.  I find that most transgender patients have very reasonable expectations for surgical results.  They want what every cosmetic surgery patient wants, to look like a better version of themselves.  When transgender patients are rejected by medical practices they often turn to dangerous procedures such as black market injection of industrial grade (non-medical) silicone, PMMA, and other substances. These injections are frequently performed in hotel rooms or in people’s houses or apartments and there is no guarantee as to what type of substance is being injected.  The clear liquid in those syringes could be industrial grade silicone or it could be tile caulk or even tire filler.  Many of my transgender patients have had these substances illegally injected into their faces, cheeks, lips, breasts, and buttocks with terribly deforming results.  I’m often asked to remove these substances or to improve the appearance of the affected areas but unfortunately the treatment options are very limited.

    I hope Bruce Jenner’s revelation has a strongly positive effect on the transgender community.  I hope that it improves how transgender and transsexual patients are viewed as both people and as patients in medical practices.  We cannot prevent people from turning to black market treatments such as silicone injection, but if these patients felt more comfortable walking into a physician’s office and discussing their concerns then they would be much less likely to seek illegal body and facial modifications.

    Rhinoplasty (Nose Job) Surgery Atlanta | Plastic Surgeon | SurgeryFor people with darker skin pigments keloid scarring can be an ugly fact of life.  A keloid is an overgrowth of scar tissue at a site of injury.  Keloids can occur from trauma, burns, surgical incisions, lacerations, piercings, and even tattoos.  In extreme cases keloids can even form after a simple abrasion to the skin.  Imagine scraping your elbow or knee and forming a a large painful keloid scar afterward!  Again, this is an extreme example but this can happen.  The good news is that keloids are treatable.  The bad news is that it’s not always an easy fix and in many cases it requires multiple treatments and/or procedures.

    Not all unsightly or dark scars are keloids.  A widened scar is simply that, a scar which has stretched or expanded because it is over a moving joint (such as the knee or elbow) or because it’s in an area of tension.  Most widened scars are not keloids therefore they won’t be treated the same way keloids are treated.

    In a keloid scar the collagen which helps repair the cut skin edges grows too much and the scar will appear swollen, hard, and sometimes painful.  Because a keloid is first and foremost an inflammatory reaction one of the first-line treatments is a steroid injection.  Kenalog steroid is injected directly into the keloid scar and after a few weeks the scar will appear flatter and lighter in color.  If this injection is repeated every month then in most cases the patient will see a significant improvement in the keloid scar.

    In an ideal situation the keloid shrinks and softens after kenalog injections.  Often as it shrinks I am able to excise the keloid and re-close the incision.  Patients who have very bad or large keloids sometimes require a single dose of radiation therapy after excision to prevent the keloid from recurring.  This single dose of radiation has been very effective in preventing or slowing recurrence but it is only necessary for large or refractory keloid scars.
